Before you can complete an application, all surrogates are required to submit our prescreen quiz to see if they’re qualified to become a gestational surrogate.
If you qualify, you’ll complete an initial surrogacy application consisting of approximately fifty questions about your background, pregnancy history, obstetrical/gynecological information, and spousal information.
After you complete the application, you’ll be required to submit specific documentation including recent photos of yourself and your home, your insurance cards, and a valid driver’s license.
We will conduct a psychological evaluation consisting of a PAI personality test to ensure that you’re prepared to handle the pregnancy. In addition, you and your spouse/live-in partner (if applicable) will undergo a background check to ensure that you have no prior convictions that could negatively impact the surrogacy process.
You’ll need to submit your medical history and pregnancy medical records for review by a reproductive endocrinologist.
The fun begins! We’ll propose potential matches based on the criteria you and the Intended Parents provide. When we find a potential match, you’ll have the opportunity to connect with the proposed Intended Parents. If both parties would like to continue working together after the call, you’ll move to the contract phase of the process.
Once you find your perfect match, a surrogacy contract will be drafted and signed by both you and the Intended Parents and the journey of a lifetime officially begins!
